25-Hydroxyvitamin D (25-OH Vit.D) is the metabolite that is usually measured and reflects the amount of the vitamin in the body. Its determination serves to detect deficiency or elevated levels. More than 95% of Vitamin D measured in laboratory tests is 25-OH Vitamin D3, and the remaining minimal part is due to 25-OH Vitamin D2. The half-life of 25-hydroxyvitamin D in the blood circulation is 2-3 weeks, i.e. it is a stable depot reflecting the level of the vitamin in the body.
Vitamin D stimulates Ca absorption in the small intestine and increases Ca reabsorption through the kidneys. It plays a role in bone formation and remodeling, so vitamin D deficiency is a common cause of secondary hyperparathyroidism (increased PTH). Severe vitamin D deficiency leads to the development of rickets in children and osteomalacia in adults.
