General information:

The concentration of SHBG in the sample is measured. SHBG testing is particularly important in cases of suspected androgen excess that is not demonstrated by testosterone levels. Sex Hormone Binding Protein (SHBG) is a blood transport protein for the transfer of testosterone and estradiol. It is a large glycoprotein (95kDa) consisting of two identical subunits. It has a half-life of about 7 days and is mainly synthesized in the liver. Its synthesis is regulated by the level of estrogen and testosterone.

Elevated levels of SHBG can be seen in:

  • adult men
  • hyperthyroidism
  • cirrhosis
  • it can also increase when taking contraceptives and antiepileptic drugs
  • during pregnancy, due to the increased levels of estrogens (physiological)

Decreased levels of SHBG occur in the following conditions:

  • hypothyroidism
  • Polycystic Ovarian Syndrome (PCOS)
  • obesity
  • hirsutism, increased levels of androgens
  • alopecia, acromegaly
