General information:

Insulin is formed in the B cells of the islets of Langerhans from the precursors preproinsulin and proinsulin. The pancreatic islet apparatus maintains a constant basal insulin secretion, which is usually about 1 U of insulin per hour. This secretion reaches its lowest values between 2 and 4 o'clock, then increases and the highest values are between 6 and 8 o'clock in the morning. Determination of the basal level of insulin in plasma or serum has little informative value for the diagnosis of diabetes mellitus, since it cannot reveal the secretory capabilities of beta-cells and the dynamics of their secretion.
